Level 4 Site Supervision Evidence: What You'll Need

7 min readUpdated 20 April 2026

The Level 4 NVQ is assessed entirely through evidence gathered from your real work activities. Because the Level 4 covers broader supervisory responsibilities than the Level 3, the evidence needs to reflect planning, coordination, and management alongside on-the-ground supervision. This guide explains what you will need.

The Four Evidence Methods

  • Evidence for the Level 4 NVQ must be gathered across four methods: direct observation by your assessor, witness testimony, professional discussion, and documentary evidence.

  • All four are required. Together they demonstrate both your practical competence and your understanding of why you do what you do.

Observation

  • Your assessor will attend site and observe you performing supervisory activities at a Level 4 standard.

  • This may include running a pre-start briefing, conducting a site walk with a health and safety focus, or overseeing the coordination of multiple trade activities.

  • Observation at Level 4 focuses on decision-making and coordination, not just task-level supervision.

Witness Testimony

  • Witnesses should be people in a position to comment on your supervisory performance at a senior level — a contracts manager, project manager, client representative, or principal designer.

  • Testimonies should be specific: they should describe activities they observed you carry out, not just confirm your job title or general competence.

Professional Discussion

  • Professional discussions at Level 4 go beyond describing what you do — assessors will probe your reasoning, your understanding of legal requirements, and how you manage competing priorities.

  • Discussions are structured but conversational. They are recorded as part of your portfolio.

Documentary Evidence

  • Strong documentary evidence for a Level 4 NVQ includes short-term programmes or look-ahead plans you have produced or contributed to, method statements and RAMS you have reviewed or briefed to operatives, permits-to-work you have authorised, site induction records, quality inspection checklists, toolbox talk registers, plant pre-use check records, meeting minutes from coordination meetings you have led or attended, and correspondence with clients, designers, or subcontractors relating to site supervision.

  • Evidence should be authentic site documentation — not prepared specifically for the NVQ — and should clearly show your involvement.

  • A brief written context note attached to each document, explaining your role in the activity, helps your assessor map it efficiently to the relevant competence units.

Tips for Efficient Evidence Gathering

  • Keep a simple site diary from day one — daily notes on what you supervised, who you briefed, and any decisions you made are a valuable evidence source.

  • Photograph activities with context notes — for example, a photo of a welfare facility inspection with a brief description of what you checked and why.

  • Tell your assessor about upcoming activities so site visit timing can be planned around key events like starts, inductions, or complex lifts.

Frequently Asked Questions

Yes. Level 4 evidence should reflect planning, coordination, and management responsibilities rather than solely direct operative supervision.

Evidence from previous sites and projects can be used if it remains relevant to the current assessment period. Your assessor will advise on what is acceptable.

Copies of genuine site documents are acceptable. Documents should not be created solely for the NVQ — they must be authentic records of real work activities.

Discuss this with your assessor early. In some cases, a witness testimony from the person who holds the records, combined with observation, can be used to supplement the documentary evidence.

Yes, provided the person is in a position to meaningfully comment on your supervisory performance. A subcontractor foreman or site manager who regularly works alongside you can be a suitable witness.
